3. Use the word “Petitioner”. Be sure the “P” is capitalized.
4. Use the word “Respondent”. Be sure the “R” is capitalized.
5. Use these exact words, all in capital letters: FINAL DISSOLUTION OF MARRIAGE DECREE.
